## A builder for the forms you actually send

Forms are built from a set of field types — including required and multiline text — so a form matches the exact questions a practice needs, whether that is a new-patient intake sheet or a short consent form for a single procedure.

## A link that behaves like paper

Each form is shared as a public link with a three-day expiry, and it accepts one submission. That mirrors what a paper form does — it gets filled in once and handed back — without a patient having to sign up for an account first.

## Signed, not just typed

A signature field lets a patient sign directly on the form, which matters for consent documents where a typed name is not enough. The signed response is stored with the rest of the submission.

## Answers that land on the record

Selected fields can bind directly to the patient's profile, so a phone number or address entered on an intake form updates the [patient record](/features/patient-records) automatically rather than sitting only inside the form response. Every submission is also viewable in full from within the app.

### How is patient data protected?

Patient records are encrypted in transit and at rest, files are stored privately and served through short-lived signed links, and every change is written to an audit log. Staff accounts sign in with passkeys rather than shared passwords.

### How much does HyperCRM cost?
