# One record, kept honest over time

Every patient in HyperCRM has demographics, a status that moves through your practice's lifecycle, colour-coded tags for quick filtering, and clinical notes that keep their full revision history — so an entry can be traced back to who wrote it and when, not just what it says now.

## Demographics and status in one place

A patient record holds contact details, country and postcode alongside a status — active, in recall, or completed — with timestamps for each change. Colour-coded tags can be applied one at a time or in bulk, so filtering a patient list does not depend on remembering a name.

## Notes that survive a dispute

Clinical notes keep their full revision history: an edit does not overwrite what was there before, it adds to the record, so a note from months ago can still be traced to a person and a moment. Notes can also flag a next appointment directly against the patient.

## Told how a patient wants to hear from you

Each patient carries their own notification preferences for email and SMS, which the [reminders](/features/reminders) system reads before sending anything — a preference set once is respected everywhere, not just in the screen where it was set.

## Where it fits

Every practice keeps the same record. [Dental practices](/for/dental) see patients and chairs rather than generic contacts and resources, but the record underneath — notes, tags, status, preferences — is exactly the system described here.

## Common questions

### How is patient data protected?

Patient records are encrypted in transit and at rest, files are stored privately and served through short-lived signed links, and every change is written to an audit log. Staff accounts sign in with passkeys rather than shared passwords.

### Which kinds of practice is HyperCRM for?

HyperCRM suits appointment-based practices that keep clinical records: dental, medical, surgical, physiotherapy and aesthetic clinics. They all run the same system, using the words their own field uses — and dental practices additionally start from a service catalogue that is already filled in.

### Do clinical notes keep a history of edits?

Yes. Every clinical note keeps its full revision history rather than being overwritten, so a note written months ago can still be traced to the person who wrote it and the time it was written — useful if a record is ever questioned later.

### Can I tag and filter my patient list?

Yes. Patients can carry colour-coded tags, applied individually or in bulk, and the list can be filtered by tag or by status such as active, recall or completed. It is built for practices that need to find a group of patients quickly, not just one at a time.
