Building, sending and reviewing forms

This guide covers building an intake or consent form, sharing it with a patient who has no account, reviewing what comes back, and having selected answers write straight onto the patient's own record.

Build the form

Add fields to a form from the available field types, including text, multiline text, choice fields, required fields and a signature field. Build the form once for a purpose — a new-patient intake sheet or a consent form for one procedure — and reuse it for every patient who needs that same thing.

Share a form as a public link. The link expires three days after it's created and accepts exactly one submission, so it behaves like a paper form handed to one patient rather than a reusable public page. The patient does not need an account to fill it in.

Capture a signature

If the form includes a signature field, the patient signs directly in the form rather than typing their name. This matters for consent documents specifically — the signature is stored as part of the submission.

Bind answers to the patient record

Specific fields on a form — commonly contact details — can be bound to fields on the patient's profile, so submitting the form updates the patient record automatically. Fields that aren't bound stay only in the form response.

Review what came back

Every submitted response can be opened and reviewed in full from within the app, including any signature. If a link expires before a patient submits it, send a fresh one — an expired link cannot be reopened or extended.

Common questions

Patient records are encrypted in transit and at rest, files are stored privately and served through short-lived signed links, and every change is written to an audit log. Staff accounts sign in with passkeys rather than shared passwords.

The link stops working three days after it was created. There is no way to reopen or extend an expired link, so the fix is to send the patient a fresh one — a new link starts its own three-day window and still accepts only a single submission.

Any field you choose to bind, typically contact details like phone or address. Binding is set when the form is built, not by the patient filling it in, so a practice decides in advance which answers should update the record and which stay only in the response.
